Blue Diamond Care is a North Carolina home care agency preparing for launch and seeking its first Per-Diem RN Service Supervisor — Home Care.
This professional nursing role supports the safe, consistent delivery of home care services through assessment, care planning, caregiver readiness and supervision, client follow-up, documentation, consultation, and collaboration with agency leadership.
About the Opportunity
This is not a bedside shift nursing position and is not a skilled home-health treatment role.
Instead, it is an opportunity for an experienced RN who values thoughtful home-care practice, independent professional judgment within defined authority, strong documentation, caregiver support, client safety, and high-quality service delivery.
The position offers the opportunity to help establish a well-organized home-care program while focusing on the professional responsibilities of the Service Supervisor role.
What You'll Do
As the Per-Diem RN Service Supervisor, you will:
Conduct initial in-home assessments for clients receiving In-Home Aide / personal-care services.
Evaluate functional and home-care needs within your professional authority and assigned responsibilities.
Develop, approve, review, and update plans of care and service instructions consistent with agency policy and professional judgment.
Evaluate caregiver competency and assignment readiness when required by the approved service model.
Provide professional supervision, consultation, and support to caregivers providing In-Home Aide services.
Serve as the designated supervisor for Companion/Sitter services under Blue Diamond Care's launch model.
Conduct client and caregiver supervisory follow-up activities and document findings appropriately.
Identify and escalate safety concerns, service concerns, changes in client needs, documentation issues, and scope-of-practice concerns.
Communicate professionally with clients, families, caregivers, and the Agency Director, and coordinate with other involved providers when appropriate to the supervisory role.
Maintain timely, accurate electronic documentation and required records.
Support continuity of supervisory responsibilities through appropriate communication and handoff when availability changes.
What We're Looking For
Required Qualifications
To be considered for this position, applicants must have:
Active, unrestricted Registered Nurse license or authority valid for practice in North Carolina.
Minimum 2 years of professional RN experience.
Graduate of an accredited nursing education program.
Ability to perform the essential functions of the position, with or without reasonable accommodation.
Ability and willingness to perform in-home field responsibilities.
Ability and willingness to travel throughout Cary, Raleigh, Apex, and Morrisville.
Valid driver's license.
Reliable transportation.
Personal automobile insurance meeting applicable legal and agency requirements when using a personal vehicle for agency business.
Ability to provide at least 10 hours of recurring weekday availability per week, reasonably distributed across weekday blocks that support agency operational needs.
Ability to communicate professionally with clients, families, caregivers, and agency leadership.
Ability to use electronic documentation and agency technology systems.
Willingness to supervise both approved Blue Diamond Care service pathways within the boundaries of the Service Supervisor role.

Prior Service Supervisor experience is welcome but not required. Prior home-health titles, hospital experience, or management titles are not required.
Experience conducting client assessments, developing or reviewing plans of care, supervising direct-care personnel, evaluating caregiver competency, and coaching staff is particularly valuable for this role.
Schedule & Availability
This is a Per-Diem (PRN) opportunity.
Weekly work hours are not guaranteed and will vary based on client and agency needs.
Blue Diamond Care is seeking an RN who can provide approximately 10–15 hours of recurring weekday availability per week, with a minimum requirement of 10 recurring weekday hours. Specific availability blocks will be established collaboratively based on agency operational needs and the selected RN's availability.
Routine nights and weekends are not anticipated as part of the launch schedule.
This position does not require unrestricted standby availability or continuous on-call coverage.
Compensation & Field Support
$42–$45 per hour
W-2 employee status
Per-Diem (PRN) scheduling model
Authorized compensable work is paid in accordance with Blue Diamond Care employment policies
Qualifying business mileage is reimbursed in accordance with agency policy
Company-issued mobile technology and field-support resources are provided as appropriate to the role
